To obtain a detailed picture of you as an individual, including your unique health concerns; your genetic and environmental predispositions; and your health goals, Dr. Kuzniar will conduct a complete naturopathic health
assessment. This visit will last one and a half to two hours. After your visit, Dr. Kuzniar will review and synthesize your case and develop a personalized program of naturopathic care to be discussed at your second visit.
You can prepare for this visit by downloading the appropriate intake form and taking some time to complete it as thoroughly as possible.
Please bring all current medical information and laboratory results to your visit. You can request these from your medical doctor.
In addition, please bring along the bottles of all supplements and other remedies which you are currently taking.
Follow Up Visits:
During your second visit, Dr. Kuzniar will explain the customized wellness plan which has been developed to address your specific health concerns. It is important that you feel comfortable with each of the steps involved in your care. During this visit, all of your questions and concerns will be addressed.
Subsequent visits will be scheduled on an as-needed basis to monitor your progress and modify your treatment plan as your health improves. The frequency and length of these visits will depend on your case and the treatments required.
Dr. Kuzniar encourages her patients to come in for periodic visits in order to ensure health maintenance and focus on disease prevention.
Although in-person meetings are preferred, there may be occasions in which a telephone consultation is necessary. The first ten minutes of consultation is complementary. Sessions lasting longer than ten minutes will be billed at the regular hourly rate.
Payment is due at the time of the appointment. Our clinic accepts cash, check, and all major credit cards.
In consideration to other clients and to Dr. Kuzniar, we would appreciate 24 hours’ notice for cancellation or rescheduling of appointments. You will be duly billed for late arrivals and missed appointments.
